Ride attendants & operators questionnaire

This questionnaire is anonymous and no information shall be attributed to individual attendants, operators, or controllers. The purpose of this questionnaire is to gather more information on ride behaviours as seen by ride attendants and operators.

Please answer the questionnaire in relation to one ride, or a group of very similar rides, that you attend or operate. If there is more than one ride or ride type relevant to your experience, please complete as many of these forms as you can.
