  The Health and Safety Executive (HSE) has today warned companies to ensure employees are properly trained and precautions are taken when working at height. The warning comes after a worker died following a fall through a fragile roof on a farm in West Wales.

  A construction worker has been taken to hospital after falling 50ft from scaffolding and being hit by debris.
• Power station safety fears - 13 June 2007
  Workers at a power station were meeting today to raise safety concerns following the death of a 49-year-old worker at the plant. The worker died following a 40ft fall.

  A man in his 40s was taken to Musgrove Park Hospital after he fell down a hole at a sewage works.

  A 34-year-old man has died in hospital, four days after falling through a skylight.

  A telephone engineer has been rescued by fire fighters after falling more than 20ft (6.1m) into a disused water reservoir.
